Understanding the Functions of Adrenal Cortex Hormones

The adrenal cortex, a vital part of the endocrine system, produces several hormones that play crucial roles in maintaining homeostasis and regulating various bodily functions. Aldosterone is often referred to as a mineralocorticoid, as it regulates sodium and potassium levels in the body. But what happens when there is an imbalance of aldosterone production? Primary aldosteronism can occur due to an adrenal tumor or adenoma, leading to an increase in urine excretion of aldosterone. On the other hand, secondary aldosteronism is stimulated by conditions such as kidney disease, heart failure, and cirrhosis (2).

Interestingly, aldosterone levels vary depending on a person's position. In the lying position, aldosterone levels can fall by up to 40% compared to when a person is standing upright (3). Additionally, plasma aldosterone levels typically range from 5-20 ng/dL in individuals who stand for an extended period.

Measuring aldosterone levels accurately can be challenging due to its minute presence in the plasma. However, understanding its functions and regulation is crucial for diagnosing conditions such as hypokalemia (potassium deficiency) and hyperaldosteronism (excessive aldosterone production).

The adrenal cortex also produces cortisol, another vital hormone that plays a role in metabolism and stress response. Cortisol levels typically peak at 8 am and decrease by more than 50% by 8 pm (4). In contrast, aldosterone levels tend to be higher during the morning sample compared to the evening sample.

It is essential to take precautions when performing adrenal gland tests, such as collecting a urine 24-hour sample with boric acid and refrigerating it during collection. Additionally, patients should avoid herbal medicines and medications that interfere with test results (5).

In conclusion, understanding the functions of adrenal cortex hormones like aldosterone and cortisol is vital for maintaining homeostasis and diagnosing various conditions.
